1099-R Forms

If you received retirement benefits last year, you are required by law to claim them on your taxes. Every year, we will send you your 1099-R form in January. For earlier access, if you have a valid email on file at SJCERA, you can access your 1099-R through our online portal.

Steps to Log into Portal:
  1. Access the portal login page
  2. Enter the email you have on file with SJCERA, and the last 4 digits of your Social Security Number.
  3. If it is your first time logging in, you will be prompted to create a password. Keep this password on hand for future logins.
  4. Once logged in, you can:
    1. View the form
    2. Download as a PDF
    3. Print it for your records or your tax preparer

Where is my 1099-R?

1099-R forms are mailed out in late January. Postage will be coming from Chicago and may be subject to unexpected delays. 

I need a reprint of my form. How do I do that?

If you need a reprint of your 1099-R form for whatever reason, you can request one by emailing our office at contactus@sjcera.org. Please use the subject line "Reprint 1099-R" for faster processing.

What if I need the State/Payer's Tax Number?

If you are in need of the State/Payer's Tax Number for tax filing, please email our office at contactus@sjcera.org. Please use the subject line "Request State/Payer's Tax Number" for faster processing.
